Rural Health Plan County Profiles
Local county health departments and hospital systems conduct Community Health Needs Assessments to create a plan to improve health outcomes. Every county is not on the same cycle and each county has designed the assessment based on their county’s needs. This results in different questions being asked and different plan formats being used. Therefore, these alone could not be used to create the Maryland Rural Health Plan; rather it is one piece contributing to its development. Below is a summary of key findings from all of Maryland’s rural Community Health Needs Assessments broken down by the Robert Wood Johnson Foundation framework from page 2. MRHA used the Community Health Needs Assessments from each rural county that were available as of June 1, 2017.

Maryland SHIP Data
The goal of the State Health Improvement Process (SHIP) is to advance the health of Maryland residents. To achieve this goal, SHIP provides a framework for accountability, local action, and public engagement. Using 39 measures, SHIP highlights the health characteristics of Marylanders. These measures align with the Healthy People (HP) 2020 objectives established by the Department of Health and Human Services. More state and local level data on critical health measures are also presented through SHIP. As outlined by the three gold buttons above, all SHIP data may be viewed by: measure (e.g., early prenatal care), jurisdiction (e.g., Baltimore City), or comparison across measures.

Rural Maryland Outlook Healthcare Dashboard
The Healthcare Dashboard was developed by the Eastern Shore Regional GIS Cooperative in partnership with the Mid-Shore Regional Council and the Tri-County Council for the Lower Eastern Shore of Maryland, and funded by the Rural Maryland Prosperity Investment Fund administered through the Rural Maryland Council. The dashboard provides Maryland’s decision-makers with information on the health care industry and prominent health conditions in Maryland. It utilizes data from the Center for Disease Control and Prevention, ESRI Demographic and Business database, and ESRI Consumer Spending database. Users can analyze data both spatially and temporally as well as filter data by primary cause of death, provider type, or spending category.
